Medical Social Worker (Homecare)

Job Description · Explore and provide psycho-social/emotional care and support to patients and caregivers/family. · Lead in ACP discussions and completion. Complete Preferred Care Plan · Provide Bereavement Support. · Coordinate in-house Financial Assistance for needy patients. · Coordinate Medical Escort & Transport for patients. · Coordinate in referring patients for Respite Care, Nursing Home and other necessities. · Counselling/ Reconciliation · Crisis Intervention · Education, Teaching & Networking Job Requirements · Degree or postgraduate diploma in Social Work · Preferably 2 years relevant experience · Must have interest to work with elderly · Displays compassion, a positive and caring attitude towards patients, internal and external clients. · Excellent communication, interpersonal and problem solving skills · Meticulous, diligent and a good team player. · Adaptable and comfortable with changes *We regret that only shortlisted candidates will be notified.
