Job Description: Position : Visiting Nurse ( RN Coordinator of Care) The Coordinator of Care is a Registered Professional Nurse who will manage all aspects of patient care related to services provided in the home. This would include Conducting home visits, completion of all required documentation and communication with PCP. Full time schedule : Monday- Friday Full Time RN Benefits : Sign on Bonus Tuition Reimbursement annually 401K plan with company match. 4 weeks of vacation, 5 personal days, and 8 sick days per year.
